We are seeking a compassionate, skilled, and self-motivated PRN Registered Nurse to join our dedicated home care team. The RN will play a critical role in assessing client needs, developing individualized care plans, supervising care delivery, and supporting staff to ensure the highest standards of in-home care.

 RESPONSIBILITIES:

  • Conduct initial and ongoing client assessments based on agency protocols and condition changes
  • Develop and manage individualized care plans in collaboration with the Administrator
  • Perform QA visits to ensure adherence to care plans, regulatory compliance, and high standards of service 
  • Supervise home care staff, provide training, and monitor care delivery for quality and consistency
  • Maintain accurate documentation and communicate effectively with clients, families, and healthcare providers
QUALIFICATIONS:

  • Current and valid Registered Nurse (RN) license in the state of Georgia
  • Valid driver’s license and proof of auto insurance
  • Minimum of 2 years of nursing experience, preferably in home health, hospice, or community-based care settings
  • Current First Aid and CPR certifications; additional credentials in home health or geriatric care preferred
  • Solid understanding of Georgia state regulations and federal laws governing home care services
  • Demonstrated ability to perform comprehensive assessments and develop individualized care plans in line with Georgia healthcare standards
  • Strong communication, interpersonal, and documentation skills
  • Ability to work both independently and collaboratively in diverse care environments, including in-person and remote settings
  • Experience supervising and mentoring care staff, with knowledge of Georgia’s compliance standards
  • Reliable transportation and willingness to travel to client homes in Northeast Atlanta and surrounding areas
  • Flexibility to travel within the Atlanta metro area and occasionally across Georgia as needed
  • Pass a criminal background check (national and required by the state), fingerprinting and drug screening tests

 
Compensation: $40.00 - $60.00 per hour
